The Reality of Abortion in Uganda: Breaking the Silence, Saving Lives

Every day in Uganda, hundreds of women face a terrifying choice. Faced with an unplanned pregnancy, fear of stigma, and a legal system that feels threatening, many resort to dangerous, life-threatening methods.
The silence surrounding abortion in Uganda has created a public health crisis. While the law restricts abortion, it does not prohibit life-saving care. Yet, misinformation, poverty, and judgmental attitudes continue to drive women away from safe health facilities—often with fatal consequences.
We cannot afford to stay silent any longer. Here is what every woman, parent, and community member needs to know about the reality of abortion in Uganda today.
The Legal Landscape: What is Actually Allowed?Abortion
There is a widespread myth that all abortion is illegal in Uganda. This is not entirely true.
The Ugandan Penal Code Act does prohibit abortion, but the Constitution and the Uganda National Health Policy allow for abortion when a qualified medical professional determines that the woman's health or life is at risk. Furthermore, post-abortion care is entirely legal. No law prevents a doctor from treating a woman suffering from complications of a miscarriage or an unsafe abortion.
The problem is that many women do not know their rights. Fear of being reported to the police or judged by medical staff prevents them from seeking help until it is too late.
The Ugandan Reality: A Crisis of Silence
Despite the legal restrictions, abortion is a common reality in Uganda. Statistics from the Ministry of Health and various reproductive health bodies paint a stark picture:
High Incidence: An estimated 297,000 to 314,000 induced abortions occur annually in Uganda.
Maternal Deaths: Unsafe abortions account for 4% of all maternal deaths in the country—equating to approximately 189 deaths per 100,000 live births.
Hospital Burdens: At major regional referral hospitals, health workers report admitting 5 to 10 women daily suffering from severe post-abortion complications, including sepsis, perforated uteruses, and hemorrhaging.
The tragedy is not just the loss of potential life, but the loss of young women—daughters, students, and mothers—whose futures are cut short or permanently damaged because they lacked access to safe, compassionate care.
Why Women Turn to Dangerous Methods
The reasons women resort to unsafe methods are systemic:
Stigma & Shame: Unplanned pregnancies, especially among unmarried women and schoolgirls, are highly stigmatized.
Poverty: As Dr. Pebalo Francis Pebolo, a senior gynaecologist at Gulu Regional Referral Hospital, once noted, “The rich find safer options, but young girls and poor women resort to dangerous methods because they have no alternatives."
Lack of Information: Many women don’t know that modern, safe medical and surgical abortions exist. They turn to traditional herbal concoctions, physical trauma, or unqualified backstreet providers.
The Solution: Compassionate Care and Education
The solution to the unsafe abortion crisis is not to judge or punish, but to educate and provide care.
Prevention is Key: Access to family planning and comprehensive sexuality education can drastically reduce unintended pregnancies.
Seek Help Immediately: If you are experiencing heavy bleeding, severe abdominal pain, or fever following a pregnancy loss, you need emergency care. Delaying treatment can lead to permanent infertility or death.
